Medical Coverage - Your first visit to a doctor for a specific workplace injury/work illness is covered, even if your claim is not approved. ​

• For​ state fund employers (L&I manages the claims for the employer) file a Report of Accident (ROA) Form or File online through My L&I (FileFast) 

• For self-insured employers (Employers or Third Party Administrator manages the claim) complete Self-Insured Accident Report (SIF-2) with your employer. Then, complete the Providers Initial Report (PIR) with a doctor of your choice.
